Registration for Winter Sports

Registration for Winter Sports at Daniel Hand High School will be held November 11-14, 2008, from 9:30 am – 12:30 pm in the Nurse’s Office at Daniel Hand High School.  Any student wishing to participate in winter sports must have the required paperwork on file in the nurse’s office.

Required paperwork:

Current annual physical (DHHS/WCPMS)                    
Seasonal Sports Participation and Consent Form ( DHHS/WCPMS)
Athletic Emergency Information Sheet (DHHS/WCPMS)
Code of Conduct  (DHHS)
Parental Waiver/Release of Information (DHHS)0

Payment, in the amount of $75.00, made payable to DHHS.

Forms may also be picked up from the Nurse’s Office or the Athletics Office at DHHS.

Academic Eligibility Requirements for Student-Athletes

•  A student-athlete cannot, AT ANY TIME, represent the school unless he / she is enrolled in at least four courses. During the school year a student must PASS at least four courses in the trimester preceding the athletic season. No unit of work for which a student has previously received credit shall be included in those required by this rule.

•  Freshmen may compete in the FIRST trimester regardless of marks received in 8 th grade.
