Athletic Forms

In an effort to streamline the registration process and reduce the number of forms to be completed, the Department of Athletics, in collaboration with the Health Office, is pleased to present the new sports registration forms.

Parents and students need only fill out these forms at the beginning of the school year, regardless of the season in which the sport occurs. Registration fees will be due at the beginning of each season.

In order to participate in ANY sport at Daniel Hand High School a student must have these forms on file with the Health Office.

 

  1. Health Assessment Record

This form must be completed by a physician and filed with the Health Office BEFORE the first day of practice. Physical examinations expire 13 months from the date of the examination.

2.  MADISON PUBLIC SCHOOLS – EMERGENCY INFORMATION

This is a new form and replaces the Athletic Emergency Information Form and the Madison Public Schools Emergency Information Form. Please complete both pages of the form. This form MUST be completed at the beginning of the school year.   

4.  CODE OF CONDUCT CONTRACT FOR STUDENT-ATHLETES

This form should be completed at the beginning of the first sport season.

 5.  CONSENT / REGISTRATION / PAYMENT FORM 

This form should be completed EACH season and should be accompanied by the registration fee of $75.00 per athlete per season.  Checks should be made payable to "DHHS".
